PGHA Complaint Submission Form
Problems, misunderstandings, and frustrations may happen in hockey. The PGHA intends to be responsive to its members' needs and concerns. So, if a member or player has an issue, they can help solve it by following the steps outlined below. The majority of issues should be attempted to be resolved at the team level first. Please note the following: 1) The PGHA cannot guarantee complete confidentiality. The contents of this document may be shared in an effort to resolve the complaint here within. By completing the form, you agree that the PGHA may share some or all of this information in the process of resolving the complaint. 2) Complaints will be addressed according to severity, resources and safety for participants. Please note complaints are forwarded to our Disciplinary Review Committee and not to the board directly.
